About the Partner / Provider

The Center for African Education & Research, Inc. 501(c)3 is committed to providing veterans with health education to improve health and wellness. We partner with nonprofit organizations and hospitals that service veterans to help us outreach to vets to implement our programming. We engage with these organizations to understand the health needs of and social determinants impacting the health of vets in developing our health education programming targets combat and noncombat vets and includes, chronic disease management, healthy lifestyles, health risks of PTSD, overcoming the perceived stigma of PTSD, managing treatment plans and referrals, buidling supportive networks, and life management. We also develop, and train vets on the use e-health tools to increase access to health medical care and behavioral services.

The Center for African American Health Disparities Education & Research (CAAHDER), Inc. is a community-based organization whose mission is to provide sustainable solutions to health disparities and inequities impacting communities of color and vulnerable populations, such as verterans, through community engagement, partnership building, education, research, and advocacy. CAAHDER was established in 2015 by Dr. Jennifer R. Warren, who is a national provider of medicaid services (NPI #1497138705) is a Certified Chronic Care Professional (CCP), trained in trauma informed care, and in addiction medicine and behavioral health. CAAHDER increases the capacity of vulnerable populations to advocate for the reduction of place-based social inequities that impact health. In 2018 CAAHDER obtained nonprofit status to broaden its mission to include provision of community-based education. In 2020 CAAHDER joined forces with Cooper Health System and the New Jersery Hospital Association to help provide services (e.g., access and training in telehealth) and education to veterans who are a vulnerable population through the Connected Care Pilot Program. Due to Dr. Warren's leadership in public health, she has been inducted in the Honor Society of Leadership and Success in 2020.
